PAY AND BENEFITS

Annual Pay Range: $72,266.58 - $91,537.25

Hourly Pay Range: $34.743549 - $44.008292

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position. This position is classified as a Program Planner.

JOB DETAILS AND QUALIFICATIONS

Clackamas County's Health, Housing, and Human Services (H3S) Department seeks a mission-driven individual to join a dedicated team committed to integrating housing and healthcare systems as the Health and Housing Systems Integration Program Planner.

Under the direction of the Health and Housing Systems Integration Program Supervisor, the selected candidate will provide leadership in overseeing, planning, developing, and monitoring the ongoing evaluation and coordination of housing and healthcare systems integration. There will be a particular emphasis on implementing the State of Oregon's Medicaid Section 1115 Demonstration Waiver for Housing Support benefit.

The Health and Housing Systems Integration Program Planner will research status quo systems and barriers to integration. The selected candidate will also assess and develop recommendations based on input and engagement with H3S housing and healthcare staff, regional and statewide partners, local healthcare providers, and community-based organizations contracted with the county to provide housing services. This position will also support the development of inter-jurisdictional work groups and strategic planning tables involving colleagues in Metro counties, staff at Coordinated Care Organizations, the Oregon Department of Housing & Community Services, and the Oregon Health Authority.

Required Minimum Qualifications/ Transferrable Skills:*
  • A minimum of four (4) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
  • Demonstrated experience advancing racial equity in a workplace and/or community setting
  • Experience with the development and implementation of strategic plans and/or programs related to housing or healthcare
  • Knowledge of principles, methods, and procedures of local, regional and statewide planning
  • Ability to facilitate meetings and provide trainings with county and community partners
  • Must pass a criminal history check which may include national or state fingerprint records check
  • Driving may be necessary for County business. For position(s) with occasional/incidental driving, incumbents must possess a valid driver's license. TYPICAL TASKS

Duties may include but are not limited:
  • Under direction, manage the coordination of regional and statewide partners in developing and implementing new programs integrating healthcare and housing systems, including the State of Oregon's Medicaid Section 1115 Demonstration Waiver (“Waiver”) Housing Support benefit.
  • Draft and recommend program policies, strategies, and methods for integrating the Waiver housing benefit into the homelessness response system; coordinate across H3S divisions in developing recommended program policies and strategies.
  • Plan and conduct system evaluation and monitoring practices and measurable outcomes to track the effectiveness of housing/health systems integration effectiveness.
  • Research status quo systems and barriers to integration, assessing and developing recommendations based on input and engagement with H3S housing and healthcare staff; regional and statewide partners; local healthcare providers; and community-based organizations conducting contracted housing work on behalf of the county.
  • Support development of inter-jurisdictional work groups and strategic planning tables involving colleagues in Metro counties, staff at Coordinated Care Organizations, the Oregon Department of Housing & Community Services, and the Oregon Health Authority; staff these inter-jurisdictional work groups as a representative of the Housing and Community Development Division.


REQUIRED KNOWLEDGE AND SKILLS

Working knowledge of: Principles, procedures, and techniques of research, program evaluation, and analysis of program design and administration; principles and practices of contract monitoring; availability of community and contract resources; facilitation; departmental policies and procedures as they relate to stated goals and objectives; concepts and principles of human behavior, behavioral or personality adjustment problems as they relate to the client group and respective program emphasis of a specific department; office practices and procedures; evidence based practice; budget concepts, basic accounting and mathematical skills and techniques of monitoring expenditures.

Skill to: Assess and present need for service, compose and negotiate contracts, and establish procedures for effective program appraisal; analyze and evaluate situations accurately and recommend effective courses of action; present comprehensive information in either oral or written form; effectively organize and prioritize; communicate effectively in public; interpret professional material to lay individuals; conduct staff training in utilizing available programs and assess their effectiveness with departmental personnel and the public; establish and maintain effective working relationships.


WORK SCHEDULE

This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
