PAY AND BENEFITS

Annual Pay Range: $46,682.57 - $58,964.96

Hourly Pay Range: $23.939781 - $30.238443

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS

Clackamas County Social Services Division is searching for an energetic and motivated person to join the Developmental Disabilities Program as a Case Manager Aide for the Children's Team.

The Case Manager Aide will join our Team in supporting the people of our community that access Clackamas County's Developmental Disabilities Program by developing relationships, treating people with kindness, dignity and respect. They will also spend time preparing and maintaining case records on clients, monitoring and assessing client services, reviewing care plans, writing narratives for case files, updating computerized case records, fielding client and provider inquiries as well as resolving client and provider issues. The Case Manager Aide may also provide administrative support through scheduling, gathering signatures on documents, and referral for services.

We are looking for an individual who has lived and/or work experience in the field of Intellectual and/or Developmental Disabilities and has a passion for ensuring equitable access to services. A successful candidate will be engaging, creative, forward thinking, and willing to collaborate to develop and provide resources, and support to community members and colleagues at all program levels. Required Minimum Qualifications/ Transferrable Skills:*
  • A minimum of one (1) year of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
  • Experience with direct case management and/or experience supporting case management functions
  • Ability to prepare and maintain accurate and legible records
  • Effective verbal and written communication
  • Experience applying knowledge of rules and regulations regarding confidentiality and the release of confidential, protected and sensitive information and reports
  • Experience with and ability to establish and maintain cooperative relationships with service providers, agencies, and/or other members of the community
  • Proficiency with Microsoft Office software (Outlook, Word, Excel, etc.)

  • Must pass a criminal history check which may include national or state fingerprint records check
  • Driving may be necessary for County business. For position(s) with occasional/incidental driving, incumbents must possess a valid driver's license. TYPICAL TASKS
  1. Under general supervision of a Case Manager: Monitors and assesses client services; assures health and safety of individuals receiving services; reviews care plans.
  2. Assists Developmental Disabilities program with administrative duties; prepares, compiles, edits, files and scans documents to client files. Creates confidential files for recipients of DD services; completes ROI's; maintains program record keeping systems; updates computerized case records.
  3. Writes narratives for case files; fields client and provider inquiries; resolves client and provider problems.
  4. Updates and maintains records with oversight of a Case Manager Senior or Supervisor; performs file reviews of DD consumer files as required by state and federal Title XIX rules to ensure compliance. Informs Program Supervisor of trends if documents are missing, incomplete, misplaced or misfiled and if there are compliance issues.
  5. Provides casework support to Case Managers.
  6. Gathers documents for, and assists in the coordination of Individual Service Planning (ISP) meetings. May participate in ISP meetings in support of the Case Manager.
  7. Participates in Team Meetings and trainings to ensure competency in providing services to individuals.
  8. Screens client cases to determine assistance needed by clients; performs information and referral functions for other staff and clients to obtain services; provides limited technical assistance when appropriate.
  9. Interviews clients, care givers, family members and other individuals in homes, hospitals and placement facilities; assesses the general well-being and personal and community support system of clients; reports to Case Managers on the condition of clients and residences.
  10. Prepares and maintains case records on clients and service providers; monitors client case files to ensure clients receive the comprehensive care and services established by the Case Manager; verifies accuracy of information provided by clients, providers and other individuals associated with client cases.
  11. Performs data entry of client assessments and payment documents; updates and maintains records in collaboration with the Case Manager; generates reports on client assessments and payment levels.
  12. Researches and investigates community resources and possible service providers; collects program data, prepares reports and evaluates information for special studies; participates in in-service training; provides casework consultation.
  13. Manages a small caseload under general supervision; assesses clients; develops and reviews care plans; writes narratives for case files; updates computerized case records; fields client and provider inquiries; resolves client and provider problems.


REQUIRED KNOWLEDGE AND SKILLS

Required Knowledge and Skills:
  • Understanding of intellectual and developmental disabilities and how they may impact a person
  • Intermediate proficiency with office equipment, including personal computers and software programs
  • Communicate effectively, both orally and in writing
  • General understanding of equity, diversity and inclusion

Ability to:
  • Evaluate basic needs of individuals and families who experience or are impacted by intellectual and/or developmental disabilities
  • Apply basic techniques of evaluating appearance and conditions of clients, homes and residential programs
  • Develop understanding of Oregon Administrative Rules governing services to individuals experiencing intellectual and/or developmental disabilities, including but not limited to Case Management, Community Living Supports, Foster Care, 24HR Residential Services, Day Support Activity and Employment Services
  • Use techniques of meeting facilitation and conflict resolution
  • Effectively obtain information needed to determine the status of individual, family, or provider
  • Apply concepts of cultural sensitivity


WORK SCHEDULE

This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off):
  • Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
  • This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
