PAY AND BENEFITS

Annual Pay Range: $51,451.97 - $64,832.46

Hourly Pay Range: $26.385626 - $33.247415

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS

Clackamas County's Social Services Division is currently seeking a Case Manager to provide case management and support to veterans who are currently experiencing, or who have previously experienced, homelessness to obtain and maintain permanent housing. The Case Manager will also help veterans access other needed support and services to include increasing income, address physical and behavioral health needs, access veterans benefits, and various other supports depending on individual needs. In this position, the Case Manager will provide housing placement and housing retention services using a person-centered, housing first model to support approximately 20 homeless or formerly homeless individual and family veteran households who meet U.S. Department of Housing and Urban Development's (HUD) chronic homelessness eligibility criteria.

The Case Manager in this position will use a strengths-based, person-centered approach to provide supportive service for veterans and their families that are experiencing homelessness or at risk of homelessness. This includes eligibility determination, obtaining and retaining safe and affordable housing in Clackamas County and partnering with a wide variety of community resources. All services will be delivered in a manner that is culturally responsible including military culture.

Successful candidates will have a strong working knowledge of laws, rules, and regulations applicable to veteran's benefits with a background in working with veterans and the ability to assess client needs and apply them to appropriate programs, services, and entitlements. The ideal candidate will be a trained veteran with a strong background and experience in providing assistance to veterans experiencing homelessness or an imminent housing crisis.

Required Minimum Qualifications/ Transferrable Skills:*
  • A minimum of two (2) years of relevant experience that would provide the required knowledge and skills to perform the duties of the role
  • Direct case management experience or a comparable function, working with homeless and/or low income populations
  • At least one (1) year using creative problem solving, with successful results, in preventing people at high risk of homelessness from becoming homeless or helping people who have been homeless access and maintain permanent housing
  • Knowledge of State and Federal laws, rules and regulations applicable to government programs and funding guidelines, including the HUD HEARTH Act, Oregon EHA and other similar programs
  • In-depth knowledge, demonstrated by veteran status, education, training and/or working experience, of veteran needs and advocacy
  • Demonstrated commitment and experience advancing equity and diversity in a workplace or community setting
  • Must pass a post-offer, pre-employment drug test. Learn more about the County's drug testing policy.
  • Must pass a criminal history check which may include national or state fingerprint records check
  • Driving is required for County business on a regular basis or to accomplish work. Incumbents must possess a valid driver's license, and possess and maintain an acceptable driving record throughout the course of employment. Learn more about the County's driving policy
  • Per the Oregon Health Authority, (OAR 333-091-1010 & OAR 333-019-1030), this position is subject to the mandatory COVID-19 vaccine requirement. Prior to the start of employment, the selected candidate must provide proof of full vaccination or have an approved medical or religious exception in place with the County as a qualification of employment.

Preferred Special Qualifications/ Transferrable Skills:*
  • Veterans or those with military service are strongly encouraged to apply

TYPICAL TASKS

Duties may include but are not limited to the following:
  1. Assess and document program eligibility. Accept referrals from Coordinated Housing Access system. Gather documentation and information to confirm initial program eligibility and recertify annually as required by funding source.
  2. Provide housing placement and housing retention services using a person centered, housing first model to approximately 20 homeless or formerly homeless individual and family veteran households who meet HUD's chronic homelessness eligibility. Utilize language interpreter service as needed to ensure equal access to housing and support services.
  3. Establish participant rent payment levels based on participant income, total unit rent and federal, state and grant requirements. Prepare initial and ongoing rent and related payment vouchers in compliance with applicable regulations.
  4. Promote and advance equity in approach to work, initial and ongoing engagement, and in broader community as appropriate.
  5. Work closely with participants on person centered action plans to access needed services and accomplish housing stability goals, including a household budget. Revise action plans as needed to meet client needs, preferences, and values using assessment and communication skills; assist clients to receive additional needed and preferred services using knowledge of public and private resources.
  6. Coordinate with health care providers, social service personnel, county veteran service office, VA and other agencies to obtain services; reevaluate and update case plans and client eligibility on a regular basis. Advocate for participants with public and private agencies, schools and others.
  7. Maintain participant records in a timely, complete and organized manner.
  8. Serve on rotation with other supportive housing team staff as back-up for direct service staff in the event of unexpected or planned absence.
  9. Collect and submit required data for Service Point Homeless Management Information System at entry, exit, annual recertification and in the event of interim changes.
  10. Prevent emergencies whenever possible through pro-active communication, including with landlords and property managers. Respond to emergencies and negotiate resolution of problems.
  11. Report witnessed or suspected abuse and neglect of children, older adults and/or people with disabilities.


REQUIRED KNOWLEDGE AND SKILLS

Working knowledge of: Methods and techniques of strengths based case management, principles and techniques of interviewing, offering supportive services and crisis intervention; Principles and techniques regarding human development and behavior in assigned area, including military culture, geriatrics, physical disabilities, developmental disabilities, chronic mental illness, drug and alcohol abuse, and poverty; State and Federal laws, rules and regulations applicable to government programs and funding guidelines, including the HUD HEARTH Act, Oregon EHA and other similar programs; Social, health care, financial (including Veterans Administration benefits) and other community resources and agencies; Effective record keeping procedures and techniques and skill to maintain accurate and concise records.

Skill and ability to: Work effectively in a multicultural environment and to advance equity in all aspects of program delivery; Gather information through research and interviews with applicants, participants, families, care providers and others; Apply comprehensive social service evaluation and assessment methods and develop and assist in implementing appropriate action plans for people with diverse needs in diverse settings.


WORK SCHEDULE

This position is included in the County's alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off):
  • Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
  • This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department's business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
