PAY AND BENEFITS

Annual Pay Range: $51,451.97 - $64,832.46

Hourly Pay Range: $26.385626 - $33.247415

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS

The Case Manager (Mental Health Navigator) provides support and connection for adults and children to resources, programs and services related to mental health, emotional health and addiction recovery. This position will work with families receiving TANF (temporary assistance for needy families) enrolled in the JOBS Program operated by CFCC. The most critical component to success in this role is an individual who can quickly develop rapport and trust while maintaining professional boundaries and completing rigorous documentation requirements. People with lived experience frequently do well in this position.

Required Minimum Qualifications/ Transferrable Skills:*
  • Minimum of two (2) years of related case management or peer support experience that would provide the required knowledge and skills to perform the responsibilities of this position - see knowledge and skills
  • Must pass a post-offer, pre-employment drug test. Learn more about the County's drug testing policy.
  • Must pass a criminal history check which may include national or state fingerprint records check

TYPICAL TASKS
  • Provides navigational assistance and support for adults and children receiving TANF needing access to mental health, peer support services and parenting resources. Coordinates access to initial behavioral health assessment appointment and documents follow up per contract requirements. Develops referral relationships with mental health service providers and community partners to increase access for TANF participants.
  • Meets regularly with ODHS staff and partner program staff to coordinate services, staff client needs and determine availability of mental health services and other community resources to meet client needs. Works collaboratively with ODHS staff, CFCC team members and community partners to coordinate services and advocate for client needs and preferences.
  • Documents client meetings, phone calls, outreach attempts in the ODHS system, TRACS, and/or record-keeping systems per contract requirements.
  • Collects, maintains and verifies client release of information documentation. Gathers additional paperwork and documentation needed to assist in making referrals for mental health assessments and community resources. Ensures client information is maintained in a confidential manner per program and stare of Oregon guidelines.
  • Participates in on-going program development including training and assisting team members. Attends county, department, division and program meetings and trainings as required.
  • Maintain knowledge of changes in laws, regulations, program eligibility criteria, reporting requirements and resources. Provides program eligibility and information to community partners.


REQUIRED KNOWLEDGE AND SKILLS
  • Skill and ability to provide navigational assistance to participants seeking mental health services. Ability to clearly and compassionately articulate the process for obtaining a mental health screening assessment and respond to questions and concerns. Knowledge and skill to obtain required releases and documentation between client, ODHS and mental health provider per specific contract requirements. Ability to provide support for the client as requested, including waiting with client at appointment. Ability to assist in scheduling appointments and requesting referrals.
  • Ability to connect with individuals experiencing mental health, addiction, parenting challenges, poverty, housing insecurity, job loss, abuse, and other stressful life events to facilitate supports and referrals. Ability to share perspectives that promote resiliency, facilitate recovery, advocate for self-care and reduce the effects of stigma in receiving assistance. Skill and ability to deliver services utilizing the principles of Trauma Informed Care, de-escalation and harm reduction techniques.
  • Ability to assist program participants in identifying their needs and preferences for service. Experience in using negotiation and conflict resolution skills to quickly address issues and advocate for client needs and preferences with providers, funders and community partners.
  • Knowledge of professional boundaries and mandatory reporting standards. Adherence to ethical standards, code of conduct and confidentiality requirements.
  • Ability to utilize Microsoft Office products and various proprietary and public databases for eligibility, scheduling, tracking and managing due dates and data collection purposes. Documentation of client meetings will require use of multiple systems.


WORK SCHEDULE

This position is included in the County's alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off). Typical work schedule: M-Th 7:30am - 5:50pm or 7:30am - 6:00pm; 30-60 minute lunch
  • Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. This position is designated as on-site/in-person. It is expected that the selected candidate will perform duties at the specified on-site work location.
  • Applicants will need to drive a personal vehicle during the workday to multiple worksite locations (mileage reimbursement is provided for commuting during work hours). County vehicle is available for client transport.
