Salary Grade: M - $40.8654 - $58.1731 ($85,000 - $121,000)


Job Summary


Under the direction of the Multipurpose Senior Services Program (MSSP) Manager, the Supervisor, MSSP will plan, organize, develop, and implement the principles and techniques employed in a comprehensive care management model for the frail elderly. The incumbent will combine both administrative duties and clinical supervision responsibilities. The incumbent's primary responsibilities are supporting the MSSP Manager with the ongoing supervision of program operations and the performance of care management staff, the review and approval of all client care plans, and client cost monitoring.


Position Responsibilities:


  • Supervises program operations including the performance of the MSSP Social Work Care Managers (SWCM) and Nurse Care Managers (NCM).
  • Leads the program's care planning and clinical services which include conducting conferences and 1:1 supervision with care managers regarding client services, reviews documentation to ensure adherence to quality standards, legal mandates and established clinical practices, and manages crisis intervention.
  • Verifies monthly documentation of care management activities.
  • Authorizes purchases utilizing Waiver services including verification that all other resources have been exhausted.
  • Implements and evaluates the effectiveness of site quality assurance processes to ensure continuous quality improvement.
  • Reviews the MSSP program operation within the host agency and the community; identifies and recommends changes to improve service accessibility for clients.
  • Provides in-service training for all staff and recommends policy changes.
  • Resolves provider/vendor/client issues as applicable.
  • Supports care management staff with client acquisition, in-home assessments, alternative discipline, and quarterly home visits to ensure program compliance.
  • Assists Medical Authorization Assistants (MAA) during absences by ensuring service authorizations are received by vendors and carried out with client satisfaction and collaborating with members of client's care team to promote continuity of care coordination.
  • Other projects and duties as assigned.


Possesses the Ability to:


  • Supervise care management staff.
  • Establish and maintain effective working relationships with CalOptima leadership and staff.
  • Plan, organize, and direct operational activities in the MSSP program.
  • Communicate clearly and concisely, both verbally and in writing with personnel at all levels of the organization and with contacts in the community, state, and federal agencies.
  • Interface and be accountable to the Department of Health Care Services (DHCS), the California Department of Aging (CDA), and MSSP's host agency (CalOptima).
  • Demonstrate expertise and ability as a care manager, as evidenced by experience successfully carrying out these job duties.
  • Utilize computer and appropriate software (e.g., Microsoft Office: Excel, Outlook, PowerPoint, Word) and job specific applications/systems to produce correspondence, charts, spreadsheets, and/or other information applicable to the position assignment.


Experience & Education:


  • Master's degree in Social Work (MSW) or Master of Science in Nursing (MSN) required.
  • 3 years of experience in behavioral health, counseling, and care management, preferably in a Managed Care environment, required.
  • 2 years of experience working directly with the elderly required.
  • Valid driver's license and vehicle, or other approved means of transportation, an acceptable driving record, and current auto insurance will be required for work away from the primary office 20% of the time or more.


Preferred Qualifications:


  • Licensed Clinical Social Worker (LCSW) or at minimal, candidacy for Association of Social Work Boards (ASWB) clinical exam preferred.
  • 3 years of experience in MSSP preferred.
  • 2 years of supervisory experience preferred.
  • Experience in health facility, health plan, case management, home health, or hospice preferred.


Knowledge of:


  • Medicare and Medi-Cal benefits and services.
  • Medical regulations and guidelines.
  • Specified Medi-Cal home-based and community waiver programs 1915(c).
